Smokey Point Family Medicine and Athena Health

Smokey Point Family Medicine is now online! We have recently contracted with Athena Health to provide help provide you with access to your medical records. The patient portal is a secure website that allows you to manage your healthcare and health information with our office. The Athena Health Portal link will take you to a private, secure site where you can enter your username and password to get access to different elements of your patient chart. With the portal you can help keep your records up to date by updating your address and billing information, reviewing your past billing statements. You also have the opportunity to check the status of your lab results, contact us through a secure messaging system, and make updates to your medical history, and allergies.

If you are currently a patient with Smokey Point Family Medicine, but have not yet signed up to get access to the patient portal, please register here. You will need to get a unique identifying PIN number, provided by our office to get access to these records. Please call during our normal business hourse, and we will provide you with that phone number.

Although you have access to your lab results through the patient portal, it is good to call our clinic for us to help advise you with your lab results.

Using the Patient Portal as a Communications Tool

Please note thate that your patient portal is not an immediate communication tool. For urgent medical matters, please contact us during our regular office hours at (360) 659-4440. In case of a medical emergency, call 911.
